SURGICAL ASSISTANT QUALIFYING CHECKLIST

An Associate’s Degree from a 2 or 4-year institution or greater.
Successfully completed an educational program criteria:

Completion of a CAAHEP-accredited Surgical Assistant program
A registered Nurse First Assisting program (RNFA)
A Surgical Physician Assistant residency program
A full medical school physician’s degree
Worked 2000 hours within the Operating Room during last 3 years
Board Certified as Surgical Assistant
Have never been convicted of a felony or crime of moral turpitude
100% honest and forthcoming in the answers on the application
